When you submit the appointment-request form, you give us your name, phone number, email address, and any details you choose to share about the care you are looking for. We collect this information only when you send it to us — you can read the rest of the site without providing any personal information. Because what you send can describe your health and the care you want, we treat it as confidential health information.

This site uses only the essential cookies that our hosting and security provider (Cloudflare) needs to serve pages safely and reliably.
